Many underlying imbalances can lead to constant feelings of fatigue. Here are some of the common ones we see in the women we care for:
1. HPA Axis Dysfunction (a.k.a. ‘burnout biology’)
Your body has a built-in stress response system: the hypothalamic-pituitary-adrenal axis — or HPA axis for short.
In a healthy state, this system responds to acute challenges, then returns to baseline. But under chronic stress, it can become dysregulated (O’Connor, Thayer et al. 2021).
When that happens, several changes can take place. Cortisol rhythms change. Energy can feel hollow, even at the best of times. Sleep becomes problematic. Your ability to bounce back from stressful days and weeks gets harder. And even though nothing shows up on a basic blood test, the system is struggling.
This isn’t about being fragile. It’s about being human in a modern world that asks too much, too often. And if you don’t measure how your HPA Axis is responding to stress, you can’t take control of it.
2. Low Iron and B Vitamin Depletion
It sounds like an oxymoron initially, but it is true: You can eat well yet still be undernourished. There are many reasons for this.
For example, you might have poor digestive enzyme and stomach acid function. This can cause issues with absorbing critical nutrients like amino acids from protein, iron, folate, B12, zinc, and more (Filardo, Scalese et al. 2022).
Importantly, if you have upstream digestion issues, it is common to experience downstream problems like SIBO (small intestinal bacterial overgrowth) and gut dysbiosis in the large intestine (Carabotti, Annibale et al. 2021).
Sadly, many people believe that a prebiotic, probiotic, or both is the answer. These can sometimes help, but often they don’t. To regain energy and alleviate symptoms like bloating, you need to focus on the root cause: compromised enzymes and stomach acid.
3. Blood Sugar Instability
That mid-morning anxiety? The 3 pm slump? The post-dinner crash?
These can all point to dysregulated blood sugar — even if you don’t have diabetes or prediabetes. Blood sugar swings (which can be made worse by high levels of chronic stress because of cortisol on mobilising glucose from the liver) can drain your energy, affect your sleep, and mess with your mood.
